Integrated health and care in Sussex

Sussex Health and Care Partnership

The NHS and local councils have come together in 44 areas across England to develop proposals to improve health and care. They have formed new partnerships to plan jointly for the next few years.

In Sussex, our partners are working together to meet the changing needs of all the people who live in our area. We want to offer better health, better care and to ensure we make the most efficient use of our resources.

East Sussex Health and Care

Over the last few years ESHT, East Sussex County Council and our East Sussex Clinical Commissioning Groups have been working closely together through East Sussex Better Together (ESBT) and Connecting 4 You to break down barriers between health and social care to deliver the right services, in the right places, at the right time.

To strengthen this work, we now plan to formally bring these two programmes together, to establish a single health and care programme across the whole of East Sussex. We will continue to develop our established projects, initiatives and services, and we will also work on new ideas to further improve the health and wellbeing of local people right across East Sussex.

By working in such a joined-up way, we believe we are able to serve the whole of East Sussex much more effectively. This approach aligns closely with the priorities set out in the NHS Long Term Plan.
