Advice for Educational Supervisors

From August 2016 HEKSS changed the process for meeting the GMC’s requirements for the recognition and approval of trainers in Kent, Surrey and Sussex. Please visit their website, to access the handbook, which clearly explains the process which Trainers must follow in order to be approved and continue to be recognised as an Educational Supervisor

Advice for CMT supervision and preparation for ARCP

The Educational Supervisor should have a meeting with their CMT at the start and end of each clinical attachment (may do both meetings together) i.e. at least three times per year and to fill in an Educational Supervisors Report (ESR) at each of these meetings. The ESR prior to the ARCP should reflect on the trainee’s performance over the whole 12 months but will be substantiated by the previous ESRs.( If having three ESRs over the 12 months is practically not possible, then there should be at least one within three months of the ARCP that covers the 12 month period.)

When completing the ESR, the ES must view the Clinical Supervisor’s (CS’s) report and use information from it and/or comment on it. We would request that the ES identifies CS reports that are inadequate for the end of attachment report. This will help identify the need for more CS training and should be discussed with the local DME.

It is the ES’s responsibility to review their trainee’s eportfolio with the trainee to make sure that good progress is being made. If there is inadequate progress then a plan of action should be put in place with targets and a deadline with a further Educational meeting date set.

When reviewing the eportfolio, the ES should refer to the pre-ARCP checklist and guidance (JRCPTB website www.jrcptb.org.uk) summarised below, to ensure good progress. This should be a supportive and encouraging role, but also to identify trainees who are falling behind or in some difficulty at an early stage and plan action with them as above. The College Tutor will also review progress at the interim review in February and March/April and help set action plans for trainees that are behind schedule.

We recommend that the ES reads the ARCP decision aid, which highlights the requirements for the trainee and will aid them in their ES role.  A check list for the necessary requirements for a successful ARCP outcome is listed below and will be used by the TPDs to review eportfolios and for the panels at the ARCP to make their assessment.

Following the ARCP, the ES should review the outcome of their trainee’s ARCP outcome and immediately raise any concerns with the TPDs.

Following the last ARCPs and the wash-up session with an external panel member, the chair for the CMT advisory committee (CMTAC)  nationally, and also advice from the national CMTAC, there were a number of areas highlighted that we need to ensure quality throughout KSS. These are listed:

  • ACATS should include at least 5 patients to be valid and so the ES should check the details of ACATs rather than just ensuring that the minimum number of 4 consultant led ACATs have been completed.
  • There should be evidence of reflective practice. We would encourage some reflective practice during each attachment with a suggested record of at least 10 episodes during the year.
  • There should be a PDP agreed at the start of each attachment.

ES sign-off of curriculum items are essential. There are mandatory requirements for the end of CMT1 and CMT2 – see ARCP guidelines but essentially:

  1. Minimum of 4 MCRs for each year
  2. Part 1 passed by end CMT1, part 2 and PACES at end of CMT2
  3. Valid ALS each year
  4. Minimum 10 consultant SLEs per year of which minimum 4 consultant ACATs per year
  5. MSF with minimum 12 assessors, including 3 consultants each year
  6. By end of CMT1 ALL Emergency Presentations should be signed off by the ES as achieved (NB – evidence for anaphylaxis can be educational sessions rather than patient experience). These must be individually signed off by the ES.
  7. Some progress in all the Essential Procedures, Part A, including skills lab training by end of CMT1 and to be clinically independent by CMT2. The evidence for independence must include formative DOPS and then Summative DOPS, which are essential for sign off. These must be individually signed off by the ES.
  8. For common competencies, the ES should confirm CT1 level completed and evidence attached for at least 5 competencies. ES to confirm evidence attached for at least 10 competencies for CMT2. This can be justified in the comments section and group competency signed off (i.e. not all individually signed off by ES).
  9. Top Presentations: ES to confirm completed evidence for at least 11 presentations by CMT1 and all presentations by CMT2. ES should record a rating at the group competency level and provide justification for this rating in the comments section (i.e. not all individually signed off by ES).
  10. Other Important Presentations: ES to confirm completed and evidence for 15 presentations at CMT1 and at least 30 for CMT2. The ES should sample trainee’s evidence and self-ratings and record a rating at group competency level and provide justification in the comments section (i.e. not all individually signed off by ES).
  11. Clinics should be recorded in the log downloaded from the JCRPTB website CMT page and kept in the Personal LibraryThe TPDs and panels at the ARCPs will not search for this information anywhere else in the eportfolioThere should be a record of 10 outpatient clinics by CMT1 and 24 by CMT2. There should also be some evidence of satisfactory performance in clinic with Mini CEX/CbD evidence, perhaps patient survey and reflective practice. This target is due to be raised to a total of 40 clinics over the 2 years of CMT and this should be the aspiration for trainees.
  12. There should be a record of a QI project each year, delivered by the trainee and this should be recorded using the QIPAT tool on the eportfolio.
  13. A central record of Regional Training days will be kept and trainees are expected to attend 70% of these.

These are the key issues that will be checked prior to the ARCP and should be used by the ES to assess the progress of CMTs. The college Tutors will use this information at their Interim Reviews.

F3 Year

Selection Centre Training Videos Now Available

HEE in conjunction with AGCAS, have produced a number of selection centre preparation videos for doctors in training. The videos provide a series of practical and informative examples to help trainees prepare for speciality selection centres. If you are a trainee about to attend a selection centre, we strongly recommend you take a look at these videos as they will help to increase your chances of being successful.

Guardian of Safe Working Hours (GOSWH)

Dr David Walker, Medical Director is pleased to confirm the substantive appointments of two positions of Guardian of Safe Working Hours (GOSWH) namely:

Dr Nadia Muhi-Iddin – Consultant Paediatrician (covering Conquest Hospital)

Waleed Yousef – Consultant Obstetrician and Gynaecologist (covering Eastbourne DGH)

The Guardian of Safe Working Hours (GOSWH) is a new feature of the New 2016 Junior Doctors Contract. The role of the Guardian is to oversee the Safety of Doctors in Training by providing assurance on compliance with Safe Working Hours. The Post will have complete oversight of DRS4 which is the mechanism by which ESHT Trainees can Exception Report.

The Post will Chair The Junior Doctor Forum and Report to the Board Quarterly.

‘F3’ Opportunities

Many doctors choose to take a year out between foundation and further training. The STFS website now has an F3 page to provide information about local opportunities, as well as links to other relevant information. If you have any relevant development opportunities for them – for example as a simulation or education fellow with some clinical sessions – you can post them on the page using the web form.

ePortfolio – please read carefully

STFS has been advised by the UKFPO of an error with regards to the TAB forms. If a trainee has completed a TAB and has met the required number of completing the TAB assessment Foundation Administrators need to change this manually from ‘Incomplete’ to ‘Complete’. For further information on required number of tab assessments please consult the Foundation Curriculum.

E-Portfolio

Queries regarding ePortfolio should initially be directed to local trust-board foundation programme administrators. If local foundation programme administrators are unable to resolve an issue they will escalate it to the Faculty Support Team at South Thames Foundation School.
