The Proactive care team at the Trust have won a Staff Award for their service to the community. The team work across East Sussex and help patients with long term conditions manage their condition and take responsibility for their own health care.
The team were presented with their award by Chief Executive, Dr Adrian Bull.

Michelle Pinnington, Proactive Care Practitioner said: “I nominated the team as they have been amazing at trying to bring this new service to the community. They have had very good results so far with patients they see with long term conditions, and have certainly improved peoples management of conditions. It has been a bumpy road at times, but they have all worked together to get the best outcome for all involved. They have coped with structural and management changes which has not deterred their determination to bring this new service to the fore. The team support each other and are very good at sharing experiences and information. It would be great for them to get some recognition, as I feel this would give everyone a little boost of confidence”.
