The Heart Failure Virtual Ward has been recognised as making a real difference to patient care. Launched in September 2023, the service has already supported over 250 patients, helping many to avoid unnecessary hospital admissions or to return home sooner.
By managing patients in their own homes, where appropriate, the Virtual Ward helps reduce potential risks of staying in hospital and supports patients to stay mobile and independent.
Patients are reviewed daily by the Virtual Ward team, they can receive IV diuretics at home, and have their observations, weight, fluid levels, and kidney function closely monitored.
The service is led by Heart Failure Consultant Dr Hiten Patel. The patients are directly managed by Heart Failure VW Specialist Nurses Sarah Bradbury and Nicola Barrett with support from colleagues in the Community Heart Failure teams and Virtual Ward GPs. Virtual Ward nurses deliver bespoke treatment with daily reviews and careful monitoring for any changes in patient condition. This ensures timely escalation if needed.
There is a multi professional approach with additional support available from our Virtual Ward pharmacy team, physiotherapists, occupational therapists, speech and language therapists and dietitians. This model of multi-disciplinary intervention works well and ensures a holistic approach to patient care.
Feedback from patients has been overwhelmingly positive, with many expressing how reassured and well-cared for they’ve felt while being able to stay in the comfort of their own home.
The Heart Failure Virtual Ward is a strong example of how integrated, patient-centred care can lead to improved outcomes and experiences.
