Complaints and concerns - Tell us your thoughts

The Trust recognises how important it is for patients and their relatives/friends to be able to raise a complaint, and to feel it may lead to improvements in services wherever necessary.

We would therefore welcome the opportunity to hear about your experience of raising a complaint with the Trust, using the form below. If you can spare a few moments of your time to complete it, we would be most grateful. Alternatively, you can contact the Complaints Team and ask to be sent a paper version of the survey with a stamped addressed envelope.

The answers you provide are entirely voluntary and anonymous, and will only be used to help ensure we have a fair and accessible complaints process. Please do not provide specific details of your complaint or our response to you in the survey. If you wish to discuss any aspect of your complaint, please contact the named member of the Complaints Team under separate cover.

The Trust also wishes to ensure all complainants are treated equally regardless of their age, race, religion or belief, gender, gender identity, sexual orientation or disability. As part of our ongoing commitment to equality and diversity, we have included some equalities questions.
