Community Heart Failure Team

The community heart failure specialist nurses are experienced cardiac nurses with a specialist knowledge in heart failure. The aim of the service is to provide patients with diagnosed heart failure and their carers/relatives with support, advice, interventions and management appropiate to their needs and wishes in the community setting.

Who can access the service?

The service is available to adults with diagnosed heart failure who are registered with a GP in the Eastbourne, Hailsham and Seaford and Hastings and Rother areas of East Sussex.

How do I access the service?

Referrals can be made via the GP, consultants, other health care professionals and patients with a confirmed diagnosis.

Where does the service take place?

Mainly within the patient’s home, some are seen in a community clinic.

Availability of the service

Monday to Friday, excluding Bank Holidays.
