The Children’s Community Nursing (CCN) Team are a team of specialist children’s nurses, who provide skilled nursing care to babies, children and young people in the familiar environment of their own home or schools, nurseries and respite centres.
The Team aim to reduce hospital admissions for children and allow earlier discharge from hospital especially for children with complex, life limiting conditions by liaising with other care providers to establish a care package of support when required.
They are responsible for ordering all equipment and for the management of supplies needed by families to care for their child at home.
Two teams work across East Sussex
One team is based at Conquest Hospital in St Leonards-on-Sea, covering the Hastings and Rother areas.
One team is based at Eastbourne DGH covering the Eastbourne, Downs and Weald areas.
Who can be seen
- All children with a nursing need between 0 to 16 years (19 years if they have special needs).
- Most referrals come from consultant paediatricians at local hospital and specialist centres, nurses locally and from specialist centres, and parents themselves.
- Referrals are assessed by CCN team but any child who has a nursing need will not be refused.
About the team
The team consist of specialist community nurses with many years of experience working with children who have a broad range of medical needs. The team works closely with Social Services, schools, respite services, family key workers and specialist centres.
