Community Rehabilitation Hub (CRH)

The Community Rehabilitation Hub (CRH) is a community clinic running in Uckfield Community Hospital on Mondays and Wednesdays for adults over the age of 18 years with multiple needs or long-term conditions who would like to improve their physical function and wellbeing, providing short term client centred, integrated care which is tailored to meet individual needs.

The service consists of a multidisciplinary team of Physiotherapists, Occupational Therapists and Rehabilitation Support Workers within three host services: Joint Community Rehabilitation (JCR), Neurological Outpatient Physiotherapy (NOP), and the East Sussex MSK Community Partnership.

About the service

The CRH aims to:

  • Optimise and promote patient’s management of their multiple needs, neurological difficulties and/or long term conditions
  • Prevent admission to hospital
  • Identify and link in with other health care support in order to provide a timely integrated approach to care

Referral Criteria

  • 18 years old and above
  • Able to access community outpatient appointments
  • Able and willing to engage with physiotherapy, occupational therapy, rehabilitation support
  • Must be medically stable
  • Within High Weald and Lewes area (and Havens if there is a JCR/MSK need only)

How to refer

Referrals to the Community Rehabilitation Hub are received through our host services. Following assessment and triage, referrals that meet the criteria for the Community Rehabilitation Hub will be allocated to the most appropriate service pathway.

East Sussex MSK Community Partnership

The East Sussex MSK Community Partnership provides assessment, treatment and rehabilitation for adults with musculoskeletal (MSK) conditions affecting muscles, joints, bones, tendons and ligaments. This includes people experiencing pain, injury, reduced mobility, arthritis, spinal conditions and other MSK-related issues.

Referral methods:

  • Self-referral (where available)
  • GP referral
  • Referrals from healthcare professionals in primary, community and secondary care settings

Joint Community Rehabilitation Team

The Joint Community Rehabilitation Team supports adults with rehabilitation needs following illness, injury, surgery or a decline in physical function. The team works with patients in community settings to maximise independence, mobility and daily living skills, helping people to remain safely in their own homes and communities.

  • GP referral
  • Referrals from community services
  • Referrals from acute hospital teams and inpatient wards
  • Referrals from other health and social care professionals

Neurological Outpatient Physiotherapy

Neurological Outpatient Physiotherapy provides specialist assessment and rehabilitation for adults with neurological conditions affecting movement, balance, mobility and function. This includes people living with conditions such as stroke, multiple sclerosis, Parkinson’s disease, acquired brain injury and other neurological disorders.

  • GP referral
  • Referrals from hospital and community rehabilitation teams
  • Referrals from other healthcare professionals involved in neurological care

Who you can see

  • Physiotherapists who specialise in musculoskeletal therapy, neurology and community rehabilitation
  • Occupational Therapists who specialise in community rehabilitation
  • Rehabilitation Support Workers who support patients with community rehabilitation

Support may include one to one sessions with a clinician, group work and guidance on self-management.
