Joint Community Rehabilitation (JCR)

Our staff will wear a Navy-blue top often with a red JCR logo on it. All staff carry ID cards which you can request to see. If our staff use a key safe to enter your home, they will always knock so that you know who is calling.

If our staff are running late, we will endeavour to let you know as soon as we can. If a member of our staff fails to attend an appointment for any reason, we would ask that you contact your local office.

Due to our different localities, our services can differ. We are a team of Physiotherapists, Occupational Therapists, Nurse Falls Practitioners, Podiatrists, Support Workers, Associate Practitioners and Care Coordinators who work jointly with the Adult Social Care Reablement team. Your needs will be identified when we receive your referral and the appropriate member of staff allocated.

If you are attending a face-to-face appointment you will need to bring a list of your current medication, mobility aids and any orthotics you use.

You will be advised of the location of where your initial assessment will be. If you are unable to leave your home, we will come to you.

This can vary depending upon the nature and complexity of your condition and the service.

We ask you notify us at least 24 hours in advance if you are unable to attend your appointment.

If we have ordered equipment for you then you can contact Millbrook to arrange a convenient day for delivery or alternatively, we might book this in for you.

Physiotherapists generally use exercises and other specialised techniques to improve mobility whilst Occupational Therapists may introduce equipment or recommend home adaptations to help people carry out their daily activities.

Please let the team know at your initial assessment or prior to booking the appointment and your request can be considered.

The PSI groups are held weekly for an identified period of time.

Due to the demand on these spaces, without good reason, we are unable to extend beyond the identified period of time, and we ask that the attendance of classes are your responsibility.

We will signpost you to local support in order to support your self-management.

Although you will have consented to treatment, you are able to withdraw your consent at any time and decline our service. If you wish to discontinue your work with JCR, please do discuss this with your keyworker who can best advise you. If you wish to have a chaperone present at home, this can also be provided upon request. If you, however, do not attend your appointments continually without letting us know you may be discharged from our service.

You will be encouraged to take an active part in identifying your goals and also in your rehabilitation and we will support you during your time with us. Please note it is our expectation that people will respect our staff and any anti-social behaviour towards any of the JCR team is unacceptable.
