Children’s community nursing team

The Children’s Community Nursing Team are based on Kipling Ward at Conquest Hospital and in the Paediatric Hub at Eastbourne DGH.

The Children’s Community Nursing (CCN) Team are a team of specialist children’s nurses, who provide skilled nursing care to babies, children and young people in the familiar environment of their own home, school or nursery.

Two teams work across East Sussex

One team is based at Conquest Hospital in St Leonards-on-Sea, covering the Hastings and Rother areas.

One team is based at Eastbourne DGH covering the Eastbourne, Downs and Weald areas.

Who can be seen

  • Children with a nursing need that can be safely provided in the community from 0 to 16 years of age and up to 19 years for young people with a complex learning disability
  • Most referrals come from consultant paediatricians at local hospital and specialist centres, nurses locally and from specialist centres, and parents themselves
  • Referrals are assessed by CCN team but any child who has a nursing need will not be refused

About the team

The team consist of specialist community nurses with many years of experience working with children who have a broad range of medical needs. The team works closely with Social Services, schools, respite services, family key workers and specialist centres.
