Our Appointment Centre is open six days a week to support patients with Most Outpatient queries. However, not all outpatient services operate on a six-day schedule. If your enquiry relates to a specific department, please refer to your appointment letter for direct contact details or submit a general enquiry through this form.

Please complete this form at least two working days before your scheduled appointment. If the department you are attending does not appear on the list, please contact the department directly using the details on your appointment letter.

We will attempt to contact you by phone to reschedule your appointment, however if no contact is made a new appointment will be sent to you in the post.
