Please complete this form at least two working days before your scheduled appointment. If the department you are attending does not appear on the list,  please contact the department directly. The contact details should be on your appointment letter.

Our Appointment Centre is open six days a week to support patients with most outpatient queries. However, not all outpatient services operate on a six-day schedule. If your enquiry relates to a specific department, please refer to your appointment letter for direct contact details or submit a general enquiry through the form below.

If you have already missed your appointment, please do not complete this form. We will get in touch with you once your clinician has decided if further treatment is required.

Day Case or Inpatient procedure

Contact the Admissions Team directly. The contact details can be found on the letter you have received with your operation date. Please don’t use the form on this page.

After you’ve sent the form

You may have to wait a few days for a response after completing this form.

We will attempt to contact you by phone to reschedule your appointment, however if no contact is made a new appointment will be sent to you in the post.
